Mehak Rizvi

Case Manager

Mehak Rizvi joined Project Guardianship as a Case Manager in 2021 and brings with her many years of experience in serving older adults and people living with disabilities. 

Prior to joining us, Mehak was a Transition Specialist at Brooklyn Center for Independence of the Disabled where she worked with mostly older adults to assist them with their discharge from local nursing homes for their successful transition to their home apartments and community living.

Mehak has completed several academic and fellowship internships. While at the Mayor’s Office for People with Disabilities (MOPD), she provided both in-person and telephone assistance and referral information to individuals with disabilities for housing-related matters, and performed community outreach throughout New York City. She also managed Project Open House which removes structural barriers in homes and assists with the mobility of people with disabilities. She also completed internships at the Bowery Residence Committee and The New Jewish Home.

Mehak is bilingual and fluent in both English and Urdu. She received a Master of Public Administration in 2019 from Baruch College, Marxe School of Public and International Affairs, and received a Bachelor of Science in human services in 2016 from New York City College of Technology.
